Hydroxychloroquine - What Goes On?

I like watching Fox News.

Last Monday, on his radio show, Hannity interviewed a Doctor in a New York hotspot. Doctor said, he treated 350 patients of his and 150 in another area with hydroxychloroquine, antibiotics and zinc sulfate . At that time, he had had none going to hospital and none dead.

Next day Hannity had another New York doctor on who was also treating his patients with hydroxychloroquine - he was more coy about citing facts and figures, but said his results from using the anti-malaria drug on patients were positive.

On Saturday, Tucker Carlson had another doctor on describing the nightmare in New York hospitals. The Doctor then stated that he was using the anti-malaria drug as a prophylactic.

And New York Governor Cuomo was procuring 750,000 doses of the malaria drug chloroquine; 70,000 doses of its derivative, hydroxychloroquine; and 10,000 doses of the antibiotic Zithromax.

After President Trump expressed optimism over the results that French doctors had gotten with the drugs, and he had ordered the FDA to fast track testing of hydroxychloroquine, the Governors of Nevada and Michigan then passed orders preventing their doctors from using hydroxychloroquine on their patients,

These reports sort of raised a lot of questions to me like -
Would it not be a simple statistical test to see if, on contracting the virus, the population of people currently taking the drug for Lupus and arthritis have a significantly lower hospitalisation and death rate than the general population?

Would it not be a simple statistical test to compare the hundreds already treated with the drugs after contracting the virus, to an equivalent number of people who contracted the virus a few weeks prior who weren't treated?

What's up with those State Governors banning its use? and

Is the drug useless, or are New Yorkers and people here and in other countries now dying needlessly?

Then, on The Bolt Report tonight, he reported that New York health workers are now on the anti-malaria drug.
And that France and Italy now allows it to be used in some cases.
And that our health workers will be starting a trial in three weeks.

So, is this some kind of Right Wing Conspiracy in presenting the information above?
Or, does a partial cure fo rthe epidemic exist, but it doesn't work for the public ......but it is good for health workers? (Kind of reminds me of our Face Mask discussion)

Any thoughts?
Regards,
Renato

And yet another study demonstrating no significant benefit and significant increased risk of heart attack.
No there wasn't a significant risk in heart attack. There was a significant increase in cardiac arrest in patients receiving both hydroxychloroquine + azithromycin but not with either drug alone. That makes sense as both drugs lengthen the QT segment which can lead to a serious arrhythmia.
But despite that there was no significant difference in mortality between those treated or not with this drug combination. There was even a trend to lower mortality with azithromycin alone.

On top of that the patients receiving the drugs had worse respiratory findings and a higher incidence of diabetes than those given neither drug. That is they were more likely to have a bad outcome.

So, in summary, from a layperson's reading of the information, at the doses used safely historically:

i) Certainly not a miracle such that everyone with COVID should start to be treated with the compound, but having some benefit has not been completely ruled out
ii) It has not caused significant harm, when used alone, although care needs to be taken when used in combination with other drugs (as would be the case for all drug regimes?)
iii) It seems quite difficult to get meaningful results out of these studies?

Are these correct?

The problem with the heart is that Hydroxychloroquine can prolong the QT c segment on the ECG.A prolonged QT c segment may lead to a potentially dangerous arrhythmia.Azithromycin which was being used in combination with hydroxychloroquine also causes prolongation of the QT c segment so the risk is increased.

So in the trial quoted above despite the incidence of cardiac arrest the mortality rate was no worse than no drug but the treated patients had more severe respiratory function and more severe co morbidities so you would expect their mortality rate should be higher.

And yes either drug alone had far less risk of a cardiac event and the mortality was lower than no drug but the difference didn't reach statistical significance so can't be called a positive study. However if the study was say twice the number it might have reached significance but we will never know.Though other studies are going on and in the future a group of investigators will probably get together and do a meta analysis of all the trials.Though at that stage the virus may have disappeared or a vaccine produced.

So basically should only be used in a hospital and part of a clinical trial.

Almost anything the mainstream media feeds you can be disregarded. Sometimes, you gotta think for yourself and do research.

It will be interesting to see how Trump's experience with the drug pans out, especially if he subsequently becomes infected. If he survives, the drug will get the credit. If he doesn't - well maybe they will claim that he was in the placebo arm of a double blind trial.
Maybe they should be showing his ECG to the world each day so they can prove that it hasn't prolonged his QTc. His ECG would be interesting - maybe it isn't that normal to start with. A CT of his brain would be even more fascinating ( assuming he has one)...

That surprised me. Obese yes. Morbidly? Not really.

Morbid obesity is defined as a BMI score of 40 or more.

Looks like unless he stacked it on since Feb that is not not morbidly obese. Just obese.
Conley recorded Trump's height as 6'3" and his weight as 243 pounds. Last year, the president clocked in at 239 pounds, according to an assessment by his former doctor, Ronny Jackson.

The Centers for Disease Control and Prevention says that a body mass index of 30.0 or higher falls into the obese range. Based on his current height and weight, Trump's body mass index is 30.4, putting him across the obesity threshold. Obese people are at increased risk of a slew of health problems, including diabetes, heart disease and stroke.

Maybe only borderline morbidly. 😉
I'd say compared with some of the bottoms I've seen in the US he is positively glowing with good health.

Indeed. If it is confirmed Fauci is using it as a prophylactic, then I might believe it has merit to do so.
He is rather surrounded by positive people and he hasn't succumbed.

It depends on your politic and I guess the time of day/week 🤣 IIRC his BMI (for what that's worth) hovers around the line between overweight and obese. I wouldn't have thought that could be considered "morbidly" obese. Nevertheless, I am sure his medical physicians keep a close eye on him

Trump or Fauci?

If you mean Trump, then I would be guessing the secret service will have been working hard behind the scenes to keep him away from as many people as possible that have been likely to have been infected. So I doubt that he has had extended contact with Covid 19 positive people.

It seems that only when positive cases cases started showing up in the Whitehorse fringe that Trump started to take it. (He has been taking it for about 1.5 weeks).
