At the threshold to the gate, Isnt the pilot guided by ground crew and the ground crew the eyes of the pilot?

Not really. You're supposed to ensure that the entire bay is clear before you enter it. That's why there are clearly visible 'clearance' lines around the bays. Nothing is allowed to be inside those lines. Depending upon which airport you're at, the 'ground crew' may do nothing beyond turning on the guidance system.

It's a bit like parking a car. If you're getting it wrong a spectator may attempt to assist, but if you hit something, it's your fault, not the spectators.
 
From what I hear the NIGs was offline and the Captain taxied in regardless without assistance from the ground and overshot the mark, this photo shows the aerobridge to apparently be parked in its correct spot

And if the NIGs is offline, you turn in, and then stop. Own goal...

Unfortunately humans still make mistakes.Training, Technology and procedures do not eliminate mistakes from occurring. Hopefully airlines like SQ and QF are less into punishment and more into helping pilots be better by learning from mistakes.

Don't think so. Some of these cultures can be very gung-ho. I mean just charging down runways .....
