The other thing that bugs me is when you use the Manage Booking system you can still email yourself an Itinerary that lists confirmed next to each flight even though that is not the case.

I am astounded this happened to me on a forced flight change.

I find it absolutely shocking that airlines such as QF still list flights as "confirmed" in their apps / on their website, even though the flight might actually not be ticketed.

There might be some coughbersome reason why traditionally "confirmed" and "ticketed" are not the same thing, but this should matter for the passengers, as this is simply totally confusing and could be seen as deceptive.
I think airlines such as QF are simply too lazy to solve this ridiculous customer confusion. If a flight is not ticketed, the app / website should, at a bare minimum, display a large "flights not ticketed - contact us for support" banner on top of the page.

The CX app actually had a warning banner that all my flights were not ticketed and to contact customer service. At the same time Qantas has them as confirmed, and will issue an itinerary with the old ticket # to your email saying everything is fine.

So it can be done. It's just Qantas.

Emirates also have similar functionality in their MMB which warns that a flight booking has not been ticketed.

I made a change to a CR last week (actually quite happy with the agent as he suggested requesting for availability to be released in a flight that had none but did suit us a lot better and it cleared right away) but no ticket after a week.
I called in again today, a request was put to have it sorted and "you will probably get an e-mail with the details as soon as it is ticketed but it might take up to 24h. Please call us back if you don't get it".
Obviously, I know what to expect and I will have to keep wasting my time to get a simple automated task in most airlines done by QF.

Have you checked APIS is in? At least DoB.
