General Medical issues thread

lead to Compartment Syndrome
Only if the bleeding / swelling occurs in a small nonelastic compartment of the body such that the pressure of the hematoma /swelling exceeds the the blood pressure. This cuts off the blood supply to the muscle causing intense pain and becomes a medical emergency because if not acted on causes the muscle to die and potentially an amputation is necessary.

The main compartments are in the lower legs, forearms, thighs, gluteal and abdomen.
A Compartment is basically muscle surrounded by non elastic Non stretchable fascial tissue.

Some athletes get Chronic compartment syndrome after excercise/sports activity. Mostly in the lower leg where their compartments are too small. When they exercise, their muscles swell restricting its blood supply. It improves after rest. There is a surgical treatment - fasciotomy, where the surgeon makes a cut on the fascia allowing the muscle room to expand.

Fasciotomy is also done in an emergency as a limb saving procedure in acute compartment syndrome - but its more extensive

Just for interest's sake on mounjaro.
Yes, one mechanism of action is slowing gastric emptying.
There are some anecdotal stories of permanent gastroparesis after these GLP-1 drugs leading to severe issues with eating- nausea, vomiting.
Also there is Ozempic Face and Ozempic Butt - saggy face and butt partly from fat but also muscle loss (Mounjaro also implicated)

AFF members may be the first to hear of an daily oral version potentially coming in the not too distant future. Maybe 3-5 years
Currently in Phase 3 trials for Diabetes and Phase 2 for Obesity
Phase 4 is post approval surveillance

my understanding is that they are mostly if not all related to the person continuing to eat substantial meals even though they don’t feel hungry and the stomach hasn’t emptied after the previous meal.

It appears not, the ones Ive heard about the stomach does not empty at all / or very slowly. The case that Ive still had substantial amount of food material in the stomach even after just clear fluids for 24hrs and obligatory 6hr fast. The patient had ceased Ozempic a month ago due to side effects
It's anecdotal though...

Looking for some recommendations. I broke my foot 6.5 weeks ago on the left foot Stiill pain on the area before the toes (ie the pressure point where you walk) and on the whole of the left side of my foot. I am now out of the moon boot. and so every step hurts Scans have confirmed that current pain is just part of the journey,, not new injuries.
Am going overseas in 2 weeks. What is the best shoe for someone in my situation - sneaker or hiking boot? Looking for something that is going to provide maximum support and protection and hopefully "lessen the blow" of each step. Any brand recommendations would be greatly appreciated. Many thanks
Slightly different but maybe useful - my SIL had an Achilles issue and swore by her Hokas. And a friend who works on her feet all day also loves Hoka.

I have an issue with reflex. Some kind of scleroderma issue. I know I can't eat too much nor sadly drink too much because the reflux burn is horrid. And being on blood thinners I don't want to risk any erosion issues. The PPi help but not if I overeat. For me, overeating is actually a normal size meal for most people I've been facing this for a few years now and think my stomach has actually shrunk. I physically can't eat much now anyway. I just don't want it. Weight obviously not an issue.
Me too from chemo induced reflux. It’s terrible despite heaps of prescription medications. So I am eating a tiny evening meal and all my chemo weight loss has stayed off.

There are a lot of unusual people out there when it comes to weight loss I met several people who managed to put on weight even after bariatric surgery. One fellow only ate mashed potato. Two -3 spoonfuls with 250 Gms of melted butter mixed in.
However a woman went to even greater lengths. Put on considerable weight after surgery so convinced her dentist to wire her jaws. She still put on weight as all she did was sit and drink chocolate milkshakes continually.

Long term PPI's do mess with gastric bacteria and lead to poor digestion that to me "feels like" mild reflux.
I take repeat short courses of probiotics that seem to alleviate the issue.

They do indeed have their issues and also with the urinary system. It changes the acidity level of the stomach but also other parts and which higher acidity levels are required. Chasing one thing after another.

Before we moved I used to make sauerkraut which is great for gut bacteria. Gut is connected to brain via the varga nerve and there is mounting evidence of brain and body health function being better when the gut is healthy. I've heard it called a little brain. You've all heard of poo transplants, right? There is a Sydney doctor / researcher, I think he is near Leichardt (?), that has been pioneering treatment in this area for a long time. There has been success in treating autism to bi-polar. Anyway, I am a big believer in good gut health. MrsP and I take a pro-biotic every day (Lifespace). Now that cabbages have come back down in price I'll get back into making sauerkraut. I have 2 big Polish crocks. It needs to be eaten as a salad for the benefits as cooking kills the bacteria. I also make yoghurt from a powdered culture and using an incubator. Great for lactobacillus.
You can see I have an interest. :)
That said, it has not helped MrsPs reflux symptoms which now looks like it might actually be an inflamed gall bladder. She had an ultrasound that indicated this. I did some research and now we have cut down alot on fat in our diet to combat this. We've changed to low fat milk, cut back on ice-cream, cut out butter (replaced with olive oil), etc..

So not just me then? Can you please tell that to the leading endocrinologist who thinks everything can be explained by a formula.

Hospital called today. I did not accept offer for the strictly 3 shakes a day weight loss program.

They did offer further referral into another weight loss program + support.

Friends son would still have the same caloric intake after surgery. He would buy a Maccas meal of burgers and chips and simply blend them into milkshakes. Clearly people need to be better educated about eating whole foods and not the liquid stuff after surgery. It's not exactly a mystery.
