Emirates direct flights to Cairns?

Its not just Cairns, be a million or two including Townsville plus outlying areas, many would do round trip in out of capital cities like CX currently do.

A million or two might be a tad optimistic. According to last estimates of population by LGA it's about 550,000 stretching up to almost 700,000 if you include Mackay and Mt Isa in the catchment.

Having said that, tourist hot spots, such as coughet and Krabi sustain long haul services, with smaller and/or less affluent populations (although they are much closer to Europe)

:p Itami (ITM) and Kansai (KIX) are both airports in Osaka. ITM has no international flights (and the local governments want the airport to close. ITM has worse operating limits then SYD).
Narita (NRT) and Haneda (HND) are both Tokyo airports. HND has limited international slots and the airport is not part of Open Skies agreements with Japan. With all the Australian HND slots in use (QF and NH on SYD-HND), all other flights between Tokyo and Australia have to use NRT.

If anyone Qatar Doha-BNE-Cairns.

They did it with Canberra to get more Sydney flights no reason why they won’t do the same to BNE which is a major missing piece of the network
The Qatar air service agreement allows 21 weekly flights between DOH and SYD/BNE/PER/MEL, +7 more provided they go to another port. Their 21 flight limit is taken up by their flights to PER, MEL and SYD, and the +7 is taken by the CBR flights via SYD.
Unless the allowed flights on the service agreement increase again, or QR reduces flights elsewhere, they can not fly to BNE. They can start new service to ports that aren't SYD/BNE/PER/MEL.
With QR apparently not doing well out of ADL and only starting CBR to get additional slots for SYD, it seems unlikely they will start DOH-CNS.

Stating the obvious it's also about inbound not just the outbound.

I'd vote for EK via SIN or BKK as the likely option. A connection onto NZ seems highly unlikely.

A distant second would be DXB-CNS-BNE.

Would have to be mainly about the inbound. KBV, CNX and HKT all have services from DOH and/or DXB and the outbound from these markets, particularly KBV and CNX is probably insignificant. But they are hours closer than CNS and much cheaper places to holiday in!

Qatar Airways’ (Unrealistic?) Growth Plans: 220 Destinations By 2022 - One Mile at a Time

Qatar Airways wants to fly to 250 destinations by 2022. Specifically, they want to fly to at least 220 destinations with passenger planes, while the remaining destinations would be served by cargo planes only. It’s logical they’d set a goal for 2022, given that Qatar is hosting the World Cup then.

For reference, the airline currently flies to just over 160 destinations, so they’d need to add about 60 new destinations over the next four years, or about 15 destinations per year. That’s a lot of destinations for an airline to add.

Perhaps an airlines’ past record of adding new routes is an accurate indicator of their future performance. I’ve never seen an airline that “announces” new routes without following through on them quite to the degree that Qatar Airways does.

On one hand I commend them for continuing to move forward (which is only possible thanks to their government having faith in the airline and willing to invest a lot of money), though at the same time I’ll go on record now as saying that there’s no way they’ll serve 220 destinations by 2022.
