Air Canada RJ impact with ground vehicle, at least 2 fatalities

I seem to read near misses or hits at US airports on the news each week now.

I can’t say I recall the same 10+ years ago.

Latest is that the two pilots are confirmed deceased, a flight attendant was ejected through the front of the aircraft while still strapped to her seat and is being treated in hospital, and the ARFF truck ended up either on its side or upside down - given that they weigh around 40 tons that shows the force of the impact.

Sounds like the truck though it was at D.

Did ATC previously tell the truck to hold short of RWY4 at D?

It appears there were more than one truck - "truck and company" and ATC approved RWY crossing in front of the Jazz express (accident aircraft)
.....

Possibly the aircraft JZA646 had just landed or about to land when approval given for the more than one truck to cross the runway.
Screenshot at the time when approval given to cross - relative to the position of the aircraft.
If crossing at D (as intended?), the impact may have been worse as the aircraft speed would have been higher.

Tragical - I wonder whether the system should change and call it as Delta or Echo??

ARFF truck can be seen lying on its side in the left of the picture - looks like the CRJ T-boned it.

Some images and additional comments from ATC here

I;m having a difficult time believing that the controller said that he messed up given that the ARFF truck was on the wrong taxiway. I suspect that the investigation will find that it was a Swiss Cheese thing like most incidents. If as reported that LaGuardia runs just one controller on nightshift then that is just stupid even if it turns out to have not played a part in the incident.

Are you suggesting it's fabricated?
To me, "messed up" does not necessarily equate to 100% fault even if the controller felt that way.

...

Other outlets suggest the single front Cabin crew was ejected through the front

...
Is CRJ900 centre of gravity to the rear?

Even if wrong taxiway, surely the instructions to the Truck should have been to cross behind AC jet.

Seemed to be unfortunately a total loss of positioning by the tower.

Not totally clear to me whether some of the audio is correctly marched at this point.

Is CRJ900 centre of gravity to the rear?
Presumably the rush of PAX from the front to the overwing exits tipped the aircraft.

An added level of stress if it actually tipped while they were exiting….

resulting damage probably moved the CoG rearwards somewhat.

Presumably the rush of PAX from the front to the overwing exits tipped the aircraft.
Possibly but it's tilted despite the passengers having been evacuated

Maybe the wreckage at the front is holding up the front of the aircraft or the missing nose and front undercarriage moving the CoG rearward.
