Fees to book a Qantas partner (AA) award flight where the city is not in the QF site

Background: I didn't think this would be so hard. Need to make a one way booking SJO (San Jose, Costa Rica) to MIA (Miami). Flight is available on the AA.com website as an award booking.

Problem: QF site does not have San Jose, Costa Rica as a departure site

Assumption: I need to book this via QF phone award bookings

Question: Is the above correct? How much will it cost to book a 2 person flight (i.e. what does QF charge approximately for using the phone service and taxes). I found this schedule of fees but didn't know if it related to award bookings where the cities are not available on line.

A follow on - if i need to book 1 or 2 more of these (i.e. similar circumstances) is it one fee for the entire call or per segment?
