I'm assuming that none of these things happened, even the last; since they flew without having to pay.The ticket could have been cancelled (any one of a number of things including wrong day, payment failure, credit card used to purchase subsequently being reported lost or stolen), but reissued by the service desk.
WRT the last, I don't see airlines being that benevolent.
