Starting from when I had to continue a course of rabies inoculations after being bitten by a monkey in Vietnam on a sea kayaking trip, through to getting all the inoculations (plus a bag full of medicines as we were going to often be quite remote from care and needed to be self-sufficient) for an African self-drive 4WD trip from Nairobi to Victoria Falls through my last trip to Nepal.

They have a dedicated travel nurse, specialised medical kits and as they inoculation regularly they are often cheaper than your local Dr who might have to order in inoculations just for you.

It is sensible to get vaccinated against Hep A(2 vaccines 6 months apart will give you life long immunity) if traveling anywhere out of developed world. Typhoid is rare but can be very severe illness. Vaccine gives protection for 3 years and costs about $50

Nothing is essential but highly recommended. I'd get the vaccines, hep A is lifelong once you have both.

With regards to Giardia, it's impossible to tell via symptoms and all gastros can lead to dehydration. LTO's issue with the doc should not be about the gastro but possibly in not recognising dehydration, if it was present at that time.

With the tours, agree Ubud is a long car trip and as a return you'll spend hours in traffic. Try for something more local :)
 
We used this guy, big aircon 4wd, good english and sense of humour and organised our tour with no animals as we requested, left 9am returned around 5pm, did wood carvers, waterfall, rice paddies, volcano, lunch and UBUD, was great day out, was a mere $50!. His web site email form is not working (we just booked him via his email for next month, if need it private mail me.

Only problem I have now is that with two and a bit months to go before the trip, I am still having great difficulty sitting in a car for much more than half an hour before my back starts aching, and it is aching quite annoyingly at the 40 minute mark - which is as far as i've gone since my operation. I'm hoping for a miracle improvement between now and then, but am somewhat pessimistic. Still. if I get a tad better, I suppose that for $50 I could hire him for as long as we can handle the trip with him, then give him the rest of the day off.
