USA Doctors and medical reports/doctors Certificates help/advice

I was unable to fly due to my ears and changed my flight - admittedly before I had seen a Dr (less than 3 hours though) but that was purely due to timing and knowing the pain and discomfort I was I there was no way I could see any Dr saying it was ok for me to fly.

The doctor said he would do a report for me for work which I said I would also use additionally for insurance.
He then came back with all my scripts and sent me to pay
When I was paying I said I only have scripts but should have a report and they said it is posted.
My partner thinks they confused sick certificate with bill.
I have to go back to have confirmation that I can fly/check up before I leave.

But can someone tell me what the usual process is with US doctors notes. I am not worried about the bill, my boss is being very condescending/demanding at the moment and I am worried if I leave without the australian style can't work note that he will take steps to try to fire me (there was a case of someone providing a fake international doctors certificate not that long ago but it was for a month extra I will have a day extra - and a day that was offered to me in the first place at that) but it seems there is a crack down on it now. So I am getting very anxious over how it all works.

I couldn't hear properly and was impatient as part of my illness so I couldn't be bothered to stand and clarify when I coudln't hear properly what they were telling me anyway.
 
Not quite sure where the "extra" day fits in - but presuming this means you will be back at work one day later than you originally planned.

I an also presuming you haven't received the medical certificate from the US Dr.

I also don't know the size of the company you work for.

1. Email your supervisor (and your HR officer if you have one) and let them know briefly the date you will be back at work and that the reason relates to a medical issue. Ask them to reply to acknowledge receipt. Keep a copy of the email.

2. If you haven't received the medical certificate, the Fair Work Act obliges an employer to accept a statutory declaration. You can google the correct format for your state. You should fill out the form with a statement outlining what has occurred. You then need to have it signed by a JP or authorised person ( this varies depending on your State). When the medical certificate arrives give a copy to your supervisor.

It is almost impossible for your employer to sack you for a single day absence unless you have a pattern of taking an extra day at the end of a leave period.

However, you may have a grumpy boss so keep your head down and work hard.

this in no wy constitutes legal advise, just a run down from an HR manager.

I have contacted my manager - hes the one thats all upity about it so i have the email and have kept it

I was hired recently with them knowing that I had a lot of leave booked. In the interview the supervisor (not mine) said and wrote - "thursday, your back on an thursday so you'll need friday off then". But throughout my training etc I have said "its ok I can come in on the friday", which I shouldn't have then it would be fine :( but I did.
We are currently due back on sunday now which means I will be at work on monday if I am cleared to fly by the doctor friday afternoon for the saturday morning flight otherwise it wont be until tuesday that we are back (hawaii flying jetstar - not a flight every day etc)
I didn't get a doctors certificate but i did get an interim report with the treating doctor, the cost and the diagnosis but it doesn't say that I could not fly so it is essentially a bill when i need a doctors report/medical certificate

I now work for a big bank.

I haven't been working long enough to have any leave patterns apart from that I had a lot of leave booked before being hired.

He is only my boss until Friday then my manager changes to someone else entirely who has just come back from maternity leave so I am not sure if she will even have any idea of the days i was to have off, but i am sure her manager does. Hr potentially has me not working Friday anyway but I am unsure as there were also staff changes in that department.

Ok weight off my shoulders about being fired. Diligent worker is no problem, just hope now that I do get the all clear for sat flight as I will feel very awkward about taking any more days off but so far the ear is still blocked but not as painful, although since seeing the doctor I haven't had any real altitude changes (waikiki is quite flat, Kauai was a problem and getting from there back to oahu almost killed me) apart from coming to our 40th floor room - which is not pleasant. I stay in or out I don't come and go.

It is good to know my obligations, I was given a very specific list of things that I was required to provide bullet points and everything and an elaborate spiel on how difficult it will be for them to obtain them later if i cant get them now.
Maybe I should go in tomorrow and give the medical clinic my written consent now that my boss can contact them to confirm?
I might do that actually as that was one of his big gripes... he would need me to give written consent then it takes time and its a drama and blah blah difficult to get but I am still here. I can do it now. I don't have to post or fax it once I am back.
