Survey Response Broken [now fixed]

I received an email this morning to take a survey about engagement. I clicked the link, and entered my answers, and when I hit submit was presented with the error here:

I don't have a lot on my plate at my day job, so figured I'd tell VFF. Considering they haven't responded to my last email from over a year ago, this seems more fruitful based on past experiences.

Not much reason to think that now.

Velocity Frequent Flyer last logged on 4 Aug 2017. And they've only posted a grand total of four times in 2017 (twice in January, twice in July).
