OS travel - One parent and kids with a different surname

We did the US in July. We left Aus separately and anticipated and issue so took the birth certificate (copy)... But had no issues and didn't need the certificate but having it gave us peace of mind. We did the same about 18 moths ago without issues.
 
I was advised to take a letter signed by both parents and the birth certificate when i travelled with our 4 yr old alone under similar circumstances. Hong Kong customs asked a few questions, read the letter and let us through.
