Cargo door failure and subsequent structural failure causing 9 passengers to exit the aircraft during flight from HNL.
Aircraft repaired
Issues surrounding cargo door design for large commercial aircraft were known for a many years prior

American Airlines Flight 1​


On March 1, 1962, s Boeing 707 feparting New York rolled over and crashed into Jamaica Bay two minutes after takeoff, killing all 87 passengers and eight crew members aboard. A Civil Aeronautics Board investigation determined that a manufacturing defect in the autopilot system led to an uncommanded rudder control system input, causing the accident.
