Has anyone had issues/bugs getting the offer to show up? I haven't seen a single offer on my Explorer for at least 6 months, even ones that weren't targeted but Amex has been insisting that's all targeted and I shouldn't be worried. Any advice?

That sounds wrong
The ones that are on amexconnect always end up on my cards

I have had some problems with VC/LL/FC online card acceptance-long phone calls with an understaffed call centre result

I've used Woolworths online a few times and it's been credited :)

All you need to do is spend $3,000 or more on your Card, each month
for 4 months, and we’ll put an additional 40,000 Membership Rewards
points
in your Account. That's on top of the points you already earn
for using your Card

Tried to use chat to confirm if this is just the primary card, or across all supp cards, I think it's across all, does anyone know ?

This is a very good sign that Amex are serious about retention. Without a generous offer there's no way I'll keep my amex open past the one year point, as the $450 that normally goes to a travel credit isn't much use to me in the short to medium term.

Yes. In my case it seems these bonus points are being recouped against bonus points from a now refunded Qantas fare. It seems they recoup the standard points immediately but only recoup the bonus points against other bonus points. I’m waiting for a call back from Amex to confirm this.
