I snapped a photo of this recently. Always gives me a chuckle when I see it. The guy who did this was interviewed somewhere — it cost them a small fortune to do because they used expensive reflective road paint to do it.

I suppose all you need is a sleepy passenger who isn't really paying attention to capture a glimpse at the last second for the sign to have its desired effect.
