General Medical issues thread

A bit over a week ago my nephew texted me saying that his daughter was ill, and had been tested for anorexia. Its been a thing developing for a while. Clinic failed to send on the test results which would have enabled the treatment path, incl psyc. Results had missed being 'compiled' :mad:

Three days ago, in absence of results they took her to Emergency. Daughter is now on 24 hr watch, drip fed and will be taken into a child mental health facility in a few days, as she has incredible mental health aversion to food and does irrational exercise (like, at 2am). One parent taking next 6 weeks off to provide almost constant care; other parent taking the next 2 off, then see.

Daughter is 10 years old. 10 !

Moral - if I come on here with some moan or whinge, just tell me to STFU, will you please?
All the best for your great niece. Very hard as a parent/carer to deal with this. So young.

Wife had break at work at 10:15am today and went to toilet. Blood coming out in urine. Very noticeable. Calls me and I quickly reschedule my appointment for doctor today and we both go to doctor. No fever but pain in kidneys, severe bloating and going to toilet often but cannot pee.

Suspect another UTI or the same as the one 2 weeks ago. We did urine sample last week and no culture's growing so she should have been over that UTI unless it managed to hide in kidneys again. Doctor prescribed Keflex this time and she had 2 at midday.

At 2:00pm she was burning all over and crying. No shakes or chills this time but still quite bad. I gave her 2 panadol and 2 nurofen. By about 3:30pm most fever had gone and she was feeling better. Still has blood in urine. We are waiting for urine results and doctor said he will check on weekend. Wife to take 2 more Keflex soon and I will check on her before midnight to make sure she's fine otherwise we go hospital again.

This is scary stuff. This is not a minor whinge. It's not a minor ailment.

Anyway I still have issues in leg. No clot or DVT which is good. There's 3-4 issues down there and no closer to solving any of them. Exercise, swimming, are not going to help as it's not possible to do either. Too much pain. Too much discomfort.

The major issue is probably stenosis in the lower back sending pain down the nerves. Need to explore radiofrequency ablation as this pain is unbearable.

Did someone say you can look forward to retirement to enjoy life? Boy we have managed to get life totally wrong. Work all your life to make others rich and then suffer for the rest of your life. Stupid concept.

Sounds stressful John. Hope she responds fully to treatment.

They will have a long road ahead. I suggest touching base with Eating Disorders Families Australia (EDFA) and The Butterfly Foundation. Both are invaluable wealth of knowedge and support. Don’t be afraid of the hospitalisation- it is essential at this stage. But don’t wait for the public system to act in regards to long-term treatment and support. Find a suitable GP/paediatrician, psychologist and dietitian with eating disorder experience. Many have waiting lists so search now. Also, remember anorexia is not just about the food but the psychological disorder. It is a hard journey so encourage your nephew to include self care and support as a priority too.

this sounds serious - if it is a kidney stone causing these other symptoms, need medical attention (ie hospital) pronto. A very good friend nearly died from sepsis from a kidney stone. She spent 5 days in intensive care and is very lucky to be still here.

So @JohnK have they looked for renal stones. Pain, blood and renal infections are common side effects and there is an increased incidence of stones in people of northern Thai origin.
Thanks. Doctor called this morning. Confirmed bacteria in urine. Need to wait 2-3 days to see which bacteria. Curious if same bacteria as 2 weeks ago or brand new infection. This one knocked her down suddenly without warning that we had 2 weeks ago. Bacteria may have been hiding in kidney like June 2023.

Wife seems much better and I suspect 6 Keflex tablets have helped. No more blood in urine as of last night but she has vomited a few times. Mild fever this morning and I asked her to take 2 x panadol and 2 x nurofen and fever passed quickly. Her appetite also back and she got up and cooked Thai style food with chilli.

Doctor said to continue with Keflex and wait to see how wife responds. She does seem to be weaker when it comes to UTIs. Perhaps chronic issues since caesarian issues 8 years ago. Kidney came up ok in ultrasound but we haven't checked for kidney stones. I'll mention this to doctor.

The doctor also mentioned possible maintenance program to put wife on lower doses of antibiotics but to take often as prevention. This was a scary episode for both of us.
 
Getting an ultrasound in a few hours for suspected ulnar entrapment. GP thinks a nerve got pinched in my elbow while under general anesthetic on chirstmas eve. Fingers in one hand have been tingling since they put the SPC in.

Still haven't heard anything about this surgery. Need to get clearance to go back to work before I use up all my sick leave just waiting to hear something.
 
Had the ultrasound. Got in early.
They didn't see anything odd about the nerve and compared it to the nerve in my other arm. As they didn't see any inflammation, they didn't want to do the injection (and the referral said injection, if required).
So the problem is either higher up the nerve, or it's been setting down over the past two weeks since I got the referral and has almost fixed itself.
Still getting tingling, but not as much as 2 weeks ago when I saw the GP.

I had bursitis in the shoulder of that arm, which required 2 injections to get sorted out, but that was years ago (early 2019).

An update on my wife's issues.

Apparently this is the same UTI infection as last time. Apparently the bacteria is resistant to penicillin (hospital prescribed augmentin on discharge and that was 10 day course), Keflex (I think wife has responded well to Keflex) and Triprim.

So Doctor has now prescribed Norfloxacin. This is scary.

Very few medical providers upload to my health unfortunately. I go to SA Pathology here in SA, it’s the Govt one, and they do upload. Clinpath et al don’t.
I must’ve been lucky then. A couple of my providers( pathology, imaging) weren’t on for the first few years, but I think all of them are now.

One of my imaging places QScan isn’t on MHR, but they do have their own website you can logon and check all your own images. I had to do it recently, and I was amazed to see images of mine going back over 10 years which I didn’t realise were lodged there.

If nothing showing on US, then I would consider a nerve conduction study.

When did you notice it?. After you woke up from the anaesthetic?. If so then it was likely due to mis positioning of your arm during the anaesthetic and pressure in the nerve from the edge of the operating table.

That's excellent. Mine is very much a blank and have countless images completed. But now know how to get the path tests done properly. It's almost like the Drs think they pay for them and won't release them. The rheumatologist I see, and whom I really like, has marked 'do not semd to my health' but luckily SA Pathology completely ignores that and uploads anyway.

That's what the GP suspected. He did a Tinel's test. No reaction at wrist, positive at elbow.
It's the arm that didn't have the IV.

That sounds like the median nerve and ulnar nerve which is not normally due to mispositioning of arm during anaesthetic - typically that's just the ulnar nerve which gets affected - because it courses around the back of the elbow and is quite superficial to the skin so it's susceptible to external compression - the "funny bone" at the elbow is exactly the part of the ulnar nerve that gets affected.

Opposite to the arm with the IV suggests not from IV

Possibly due to pressure from whatever was holding arms in place. Its also rarely due to the blood pressure cuff going up and down

Worth asking anaesthetist what was holding arms in place.
