It was during the Howard years, but was primarily driven by the RBA. Initially, it was only large businesses that added the surcharge. The current spread to small business came later.
The ban on surcharging was typically within the scheme (Visa or Mastercard) rules. What the RBA did years ago was basically banned the schemes from having that restriction in their rules. In other countries, the schemes probably still have that restriction in their rules.
AFAIK I is used to signify a Business Upgrade. It will earn at original class in Enrich. Not sure how you would do it on a partner FF program, but I don't think it earns on any partner..,
It's more general than GST inclusive - any ascertainable amounts have to be included in the single price. It's why airlines have to include all the taxes and charges in the price, hire car companies need to include all their charges and so on.
I get the impression that stuff happens when the hotel is onboarded into Accor. Seems to me that Kintamani access was magicked up to keep the Palace exclusive. I would be pretty sure Accor is across and signed off on it.

If you're just on a dom flight - then Qantas have to have enabled it for it to be in the app. It's often for weather or other operational reasons when they do it.
Not sure what you mean, but classic rewards are usually booked under the operators code, regardless of codeshare arrangements (exceptions may apply, your mileage may vary, etc etc)
