That is a fairly big 'oops'. Although I suppose in their defense it was not a usual situation to opening a door. I hope they don't lose their jobs.

indeed... but the instruction to cross check is to ensure that directions to 'arm' or 'disarm' have been carried out... so it kind of depends what happened... was there a general instruction to 'disarm' the doors? without that, there may have been no prompt to cross-check.

many airlines have procedures in place to ensure the door is opened by ground staff from the outside (ie when stairs/airbridge is in place). Opening from the outside will - depending on the aircraft - automatically disarm the door and serves as a third check. Some doors that are manually armed (for example 737s), opening from the outside won't disarm them.

I somehow donn't think it's as easy as "putting it back in"!!!!
