We leave for the USA shortly, will probably take US$800 in cash ... have my hand out giving tips from the get go. Never seems to stop!
 
What absolutely gives me the screaming heebie-jeebies is looking at a menu that says steak $20 and then twenty minutes later getting a bill for around $30 with fed taxes, state taxes, service charge and then the hand comes out for a handsome tip for very mediocre service on top of that.

And the tax applied varies from state to state.
And we have given up trying to use change - we used to try and have the coins ready so we could pay, but it is so random it’s easier just to tip the shrapnel into a tip jar.
 
You would be wise to bring Small denomination USD as tipping is very common
