retroactive mileage claim, but city not on drop down list...

Hi all

Trying to do a retroactive mileage claim to KrisFlyer for some flights undertaken on United.

While the Krisflyer number correctly appeared on the boarding pass, I suspect the addition of a middle initial (different from the KrisFlyer account name) may have caused some problems (the flights were two months ago).

However - it was a small regional flight in the USA, and the city doesn't appear in the drop down list. The class is eligible for accrual.

Any suggestions? The pdf form seems to be for partner (non-air) claims.
