Is this premium excessive?

My 67 yo husband had a heart attack 5 years ago and with a stent and medication he has been stable since. There has been some problem getting travel insurance in recent year (appeals and added clauses scenario), mainly because he continued to smoke up until March this year.He quit and 6mths later we applied for travel insurance through Covermore (via Flight Centre) for our 35 day holiday (cruise mainly) to the Americas. No doubt the fact that he is stable and had quit smoking helped this time.
My concern is the amount of extra premium asked. It is almost $700 and we would have an excess of $400 per event, plus the maximum cover for medical and hospital treatment will be $100,000. On top of the pre-existing cover you have the standard international policy about $350 added onto that....so $1050 in total. I am 57 and was able to get a policy through Sothern Cross for around $120....a huge difference.

Should I feel relieved that (a) we can get away with some peace of mind that there is at least some insurance there ( I have no idea what the costs of hospital treatment in S.American countries or Miami, San Francisco are)
or
(b) shop around for a better price.(is $30 per day reasonable? )

I'm reluctant to bother the Doctor again if the premium difference with another insuranmce s company.
Also why does Covermore not automatically cover cancellation? I had to select my own level of cover and this increases the premium.
