The Shure ones come with a "Fit Kit" that provides three different types of sleeves, two of which also come in three different sizes. The sound isolation is produced by creating a good seal between the sleeve and your ear canal, and they also rely on this seal to provide teh full low-end frequency response. But with all the different options its easy to find one that is comfortable.

One of the types in the fit-kit is the universal foam sleeve. These are the compressible foam type like ear plugs.

You can also get custom ear molds made, which is what regular users such as professional musians do.
