Report: Qantas B744 at Los Angeles on Oct 20th 2007, burst tyre on takeoff, hydraulics failure

The crew of a Qantas Boeing 747-400, registration VH-OJB performing flight QF176 from Los Angeles,CA (USA) to Brisbane,QL (Australia) with 406 passengers and 18 crew, noticed an EICAS alert indicating, that hydraulics system #1 had failed, shortly after departure from Los Angeles. Flight attendants reported a bang in the vincinity of the left main landing gear shortly before the airplane became airborne. Air Traffic Control radioed a short time later, that tyre debris, but no other material, had been recovered from the runway.

Conflicting advice between two manuals contributed to the emergency landing of a Qantas jet in Los Angeles earlier this year, the aviation safety investigator has found.

The Sydney-bound plane, with 232 passengers and crew aboard, was moments from lifting off in late March when the pilot noticed a warning light in the coughpit.
