I've contacted hotels.com and was unable to get a straight answer. United Hotels is only contactable via phone.

I've made a booking via United Hotels (so I can earn United Miles). My cousin (in Canada) has also made a booking on my behalf via hotels.ca (so he can earn free nights).

Just wondering if I'm able to quality for United Miles on the reservation made through hotels.ca or does the booking need to be made direct through United Hotels in order to obtain the United Miles.

I hope that makes sense.

I doubt you would earn points when booking via an external supplier. United Miles and United hotels will explicitly state the earning requirements.
