I have (had ...dummy) some street cred in building teams and relationships , a great place to work
high on cameraderie and low on new age social (me me me) engineering can be
more attractive than just the hourly rate if well done.
Needs an holistic approach , meeting the multifarious needs of folks who can pay for the service.

Well I finished a hospital to home transition some 5 week ago. A totally disorganised bunch. Still looking for an alarm system which the insisted I have. The coordinator of my package picked it up at home herself 4 weeks ago.

But on the home front things have improved considerably. I am back up over 70KG so BMI just in the normal range.
The turn around with Mrsdron after a consult with our GP. He didn't pull any punches. shocked her. But that night she cooked own dinner and ate it. Her appetite isn't Great but having 3 meals a day.
Also taken up knitting again and we have been out to a restaurant.
Now our thoughts have turned to travel.
 
But on the home front things have improved considerably.

Our recently deceased friend had a home package, she had to prepare the house for the cleaner .
There was an extensive list of cannot do's , not allowed to lift or move anything.
Typical socialist bureaucratic structure , all about minimising effort and responsibility

The cooperative care idea has been floated by a bunch of friends at a certain age who live nearby . We all have enough space to house the carers if needed overnight in their own digs. Its the coordination as you say thats the sticking point.
