This is unfortunate obviously but also brings back disconcerting memories of a long and ultimately fruitless and frustrating email exchange I had with VA when they were flying the ATR-72 between SYD and Canberra. They would sometimes use the tail stand, sometimes not, and I could never tell why not. It was always much more stable with it, especially as passengers always loaded from the back, and crew would put suitcases aft of the stairs as well. I gently asked if it was VA SOP to use the tail stand (of course it was). I was also worried by passengers being directed to alight before the port side propeller was stopped and secured. I politely raised concerns about both matters - got a polite "we'll look into it" response, then crickets. When I followed up, including with photos of both issues happening on my next flight a few weeks later, I was told they'd done an investigation and were satisfied it only happened once so I should just chill our and stop worrying. I followed up again with photos of it again, and got nowhere. I wrote again, and the last I got was a 2016 email from the General Manage of VA Network Operations promising to come back to me within 7 days. Then nothing ever again. You can't fight every battle, and I had a kid born that year and got sidetracked with that. I'm a loyal VA passenger (and former GA pilot, a loooooong time ago), and I still fly VA, but this experience gave me a really bad feeling at the time and, to a degree, ever since.
