But does that cover the scenario of one being a US citizen and one not?

I am not aware of any exclusion. The CBP agents perform the same tasks and will scan every passport (US citizen included) and verify if they are eligible to enter the US. mjt57 said earlier that the CBP agent confirmed to his wife that they could have been processed together in the US citizen lane.

Yes. This is quite new (January 2014), but you still have to live at the same address.
 
It'd be getting very old fashioned and ethnocentric these days to base proof of marriage on having the same surname. Pretty much every married couple in China would fail that test for a start.

On a related note, I'm about to do something similar to the OP in October but at MEL (me being Australian, partner is Chinese). Can we use the Australian queue together or will they split us?
