Rather interestingly, my wife got an email this afternoon from EDR with the subject heading: "Mrs LW, rate your shopping experience at Woolworths {Suburb}".

The body of the email:

A Quick Survey

Mrs LW, how did we do?

It was great to see you at Woolworths {Suburb} recently.

So we can continue to make your next shopping experience even better, we’d love to hear your feedback.

A few minutes is all it takes.

Thanks for shopping with us,
The Woolworths Team.

There is a link: "Take me to the Survey".

Has anybody else received them - what did you say??

Thanks Rallo. Yes, the store she attended was a different one than normal. That probably makes sense, as most of our shopping is done through one of 4 stores of via online shopping.

I received one of these sometime last year. The following week I received another email for a different card for a shop at the same store. I filled out the survey and about a week later got a call from the store manager thanking me for my time and answering some of the questions I had asked in my feedback. It was a very interesting experience. Now that I think about it, I believe it was in the first half of 2012 that I received the survey.

The store I got the survey for was my main WW store so I am guessing that each store gets to take part in the surveys from time to time.
