What's the best way to book a Oneworld DONE4 ex Jakarta - which airline do I call?

Cathay offices seem pretty competent, so I'd certainly give the Cathay office there a try. Be aware though that every single Cathay office (with the exception of BKK) I've encountered will drop you like a hot potato if your first flight isn't with them. Other than that they all seem pretty easy with offline payment and the like.

Thanks for that. I think my first flight is with JAL, though I will be using Cathay as well. Should I try them instead or will that not work starting with Jakarta?

I assume I need to call a booking office based in Jakarta right?

AFAIK JL charges v high fuel fines (though not sure of their policy w/r/t picking up fuel fines for other carriers). I'd try the Cathay office first.

You can call:
1) An office based anywhere in Indonesia
2) An office based in an origin with lower fare levels than Indonesia
3) An office in Canada

Japan has a AA Agent, So I suggest you deal with them.

Call 0011 1 800 247 3247 and create a booking NRT-xx_-xx_...xx_-xx_-NRT.

Wait for it to be priced up. Call back 24 hours later and get price. (In Yen)
