Best travel insurance for the USA

Good Eve fellow frequent flyers,

Being that I'm heading to the US for the first time and it's not somewhere that's cheap to get oneself stitched up quickly and cheaply if travel insurance is being a pain in the bottom, would any of you recommend or stay away from particular companies or policies?

At the moment I'm leaning toward Allianz so I may collect my VFF points. I was considering an annual policy but my trip is 39 days rather than the 37 which is the limit for a personal trip.

I've previously held policies with AAMI, TID, Worldnomads and more recently an annual policy with Medibank, fortunately I've never had to make a claim
