I was just scanning it and realised it didn't seem correct.
 
At my airline, there's an underlying company "rule"(?) that honest mistakes are just that, mistakes. But if there was some sort of procedure defiance, then it's not okay.

I'd bet that this object wasn't meant to be put there in the first place. Airlines are very good at creating procedures to minimise risks (swiss cheese model). In saying that, I'm happy to be proven wrong as I don't work on the ground.
 
According to the Telegraph in London the damage was £4,000,000 STG....
