Where did the AFF Logo originate and does it mean anything or is it just a random drawing?

It was designed by consensus I believe about 4 odd years ago. There was lots of debate if I remember correctly.
 
Has the favicon recently changed?

It looks more blue than I recall, and the icon at the top of this page is more red...
