A question in relation to airport in Italy

Hi guys, I have few questions in relation to get to and away from airports in Italy.

Milan Malpensa Airport
I will live in a hotel right next to Milano P. Garibaldi. My flight is due to leave at 6.20AM. I would like to take the Malpensa Express to the airport. However, subway or tram will not start until 6AM. I have heard from a website that Malpensa Express could be running through Milano P.Garibaldi as from mid Dec. Does anyone know anything about this? Alternatively, can anyone give me an estimate on the cost of the Taxi to the Airport? An off the topic question, does most taxi driver mostly honest in Milan and will not cheat tourist?

Pisa Airport
I will arrive in Pisa Airport in late evening. Is there's a taxi rank outside the airport? Again, is most taxi driver honest in Pisa?

I've just returned from Italy: there is a taxi rank just outside the main terminal at Pisa. We had a female driver, and she efficiently and expertly took us to Piazza dei Miracoli for 10 Euros.
