It's happened out of Melbourne.....QF, passenger gets on and someone else in their seat. Crew member puts them in an empty seat as they are forceful about being on the flight and in the hustle to get the plane off the ground crew are focused on the seat number not the flight number and during the announcement while taxiing they realise they are on the wrong flight.
