American 777 loses door at D/FW

Deplaning didn't go smoothly Tuesday morning for an American 777.
Apparently, an American Airlines flight arriving at Dallas/Fort Worth Airport lost its door in a Terminal D jetbridge.
The aircraft pulled up to gate D32 around 6:20 a.m. and docked with the jetbridge. Flight attendants opened the door to start letting passengers off the plane.
However, it appears that the brakes either failed or were not set and ground crews did not put blocks around the wheels as the plane rolled backwards about 200 feet, ripping the door off the Boeing 777.

After reading this post I have just had a lil look outside to see if there are any other planes with a door missing. I'm gonna think to myself I'm safe as I'm at Terminal A... :shock:

Loose is the adjective to describe the door as it was ripped away from the frame.

Aren't wheel chocks supposed to be a safety measure in case the brakes fail? And it is probably lucky that the plane did not fail in the wrong direction, i.e. slide towards the terminal. In any case, when they realised the failure, how did they eventually stop the plane (assuming that it was the brakes and not the chocks)?

I'm calling human incompetence on this one.
