I was able to accept a minor QR flight time change through their app or website, so might be worth checking there if you haven't already? The booking was through QF and they said I had to call but was o/s at the time so went the QR route.

If you look at the recent news.com.au articles about QF stuff-ups, there is some sort-of consumer advocate that seems to have been feeding them the stories. Might be worth a try as well.

That is very odd, mine definitely doesn't mention a refund. Might have to go on chat and see how I go. Edit: I did find this note on the cancellations page: so maybe because I have used benefits for this month I am not eligible for a refund.

Assuming it is the QF tax invoice, it is just standard QF. Updates as often or less than the QF e-tickets IME.

I haven't had to call in the last few months, but my experience over a lot of calls in the 12 months before that was that there was no discernable difference in the quality of staff you connected to whether calling the 1300 or 13 number (or 131211).

As mentioned earlier, IME they always charge the full amount for the new booking and then refund the old amount. I don't think there is an option to 'just charge the difference'.

No it is not accurate, it really just means that operator doesn't know how to do the job. I would call again.

Not just oversized - it seems like every person on US domestic flights at least has a max carryon-size wheeled suitcase plus a large backpack (presumably to avoid paying for baggage and/or it getting lost).

For OneWorld, Check My Trip (usually referred to here as CMT). IME CMT will usually notify you at least 15 minutes before QF will, and sometimes QF is much, much later, if at all.
