Couldn't agree more - and as I stated earlier, any airline attempting to implement such a system would not only be outraging a lot of passengers and "current affairs" programmes, but likely dicing with anti discrimination laws as well, at least in this country.

In all honesty the answers to all questions could be yes, but in all cases you are relying on the integrity of the other passengers. The airline staff aren't bouncers, they can't throw people out for using the wrong dunny, spreading their bags in the lockers or knocking the back of the chair.
