Normally the merchant/airline accepts the letter from the bank for a different card. I haven't heard of that being denied before. Refunds to a credit card shoukd only take a few days. Refunds to BPay take much much longer.

Debit cards can be a fixed rate, so on small transaction amounts they can be more expensive than Amex to process. So your bakery could be quite accurate in what they are charging.
