First clue is that when you mouse over qf logo it doesn't allow u to click on it to go back to main qf site.
Aside from the obvious personal password requests, qf wouldn't bother asking for your credit limit, or security odes.

Blatant bull**** attempt at emptying your bank account if you are naive enough to fill out form.

Darwin's theory of Natural Selection at work here.

edit...

I have removed part of the addresses but please don't try and follow links, is just an example of what anyone can findout or check if they want to check a suspicious page out a little.

Also tried rightclick properties on image and looks like qf address but has image wriiten in address line....as in jpeg.
DONT CLIICK THIS

Then if you rightclick properties on the main part of the page it shows a web address for BelleArtEventos..
DONT CLICK THIS FFS http ://bellearteventos.com/wp-content/languages/themes/SurveyQa
Purports to be Dance academy.
I'd notify your banks Cyber Security department of this if you are with NAB

Nothing about it looks legit (I got I also). No use of name, frequent flyer number or status. Dodgy senders email.

How funny. A customer satisfaction survey asking for email password and credit card details. (I clicked on my phone).
