my wife broke her lower arm last week and had a plate inserte and pinned. we are scheduled to fly from Sydney to Lima in about three weeks and she will still have the cast on her arm then. Are there likely to be any issues with airport security screening or any advice on how to minimise these?

Shouldn't be any issues. As Sam says the scanning will pick up the metal plate. I would get a letter from the Ortho looking after her to cover any queries or questions.

Agree, letter and you could (also) take a photo of the X-ray with you (eg. on iPhone) which would show the plate.

In Sydney have her ask to go through the body scanner. No problems.through normal screening the metal plate will set off the alarm and she will have to be wanded.
The only problem mrsdrron had with a plate in her leg and moonboot was Scotland.

I'd imagine a backscatter machine would show where the plate is and that it's not a weapon.

The back scatter machine wont go deep enough anyway. The normal detector sure will though.
