I have conundrums when I book. I like Dr as a gender neutral title - I abhor the dark ages notion that a woman should be defined by their marital status. But I also don't want people asking me to help in medical emergencies if I can help it - no one wants pathologists helping patients. There are pitchforks and everything last time we tried.

I do that for domestic flights, no problem; never been game to try for international, lest it cause check-in issues like you describe.
