When requesting an upgrade on MH, is it confirmed immediately upon booking? The QF experience of not being confirmed until an hour before the flight is completely unacceptable to me, even if the upgrade is requested a year in advance.

What type of QF upgrade are you talking about here? For instance, a J to F upgrades are being notified about 48 hours out, or were last year when I did a couple. I think any airline that confirms an upgrade on application way out is cutting its own throat (as in: some Nth American airlines); a week out is all anyone should expect.

Economy to business, but too long ago to remember the actual sub-class codes. Actually I think it may be 3 hours officially. The worst example was never being told at all - my wife pointed out that we had "Business" tags attached to our luggage after checkin.

Emirates claims they will advise before making the original economy booking whether I can make an upgrade, which I must do over the phone immediately after that booking. So far I can only confirm the "negative" experience, i.e. they told me there was no chance of an upgrade on any of dozens of feasible routes, plus or minus two weeks of my preferred travel date. Don't think you need to worry about them cutting their own throats!
