Paternity Leave, how to "apply"?

Hi VFF,

Earlier this month my partner and I just brought our first baby girl into the world, it has been the busiest month of my life! Going forward I have decided to take 4-5 months off work to be with my daughter starting the 14th of April, which will be limiting my travel massively. I am wondering how to go about applying for the Parental Leave offered by Velocity? Who do I call and/or what documents do I need to get together?
