Claim Missing Points Error

Having returned from a recent USA trip that included a KUPP run to AUA one of the flight segments was mysteriously missing from my QF activity statement. No matter - just lodge a missing points claim which I did, however I've just noticed the recent addition to my activity statement is showing the missing flight but in economy not first!

Does this mistake happen often? Just a phone call will suffice?

Boarding pass and itinerary are the two likely requests.

It probably isn't in F though, should be in J. International flights to places like AUA are usually in Business.

Why don't they ask for these during the initial request?

It was a MIA-ORD leg booked into P class.

Because the systems are well connected and it can (usually) be done electronically. Occasionally that fails.

Makes sense now. Thought it was the MIA-AUA (or similar) leg that had the issues crediting.
