It's a known occurrence that's been discussed in this forum before. It isn't related to the LHR-FLR route but rather the booking class that BA sends through to Qantas after you've upgraded. Qantas see it as U I believe, which is ineligible for earn.

According to The Age the incident occurred on takeoff in FCO. Flight crew were unaware, not detected until ground inspection in Perth.

What does the second picture show? Is that just ripped tread stuck to the underside of the plane? Or has it punctured the skin of the aircraft?

Is the phone number really relevant? Most scamees do not stop to check things, that's the whole premise of scams. I doubt many people are looking up what the Qantas number really is and therefore getting fooled by the similar looking number. They're just doing what the text says.

If you can do hand luggage only, you could always "feel ill" in Shanghai and fail for board for your final leg, provided you're booking a one-way itinerary.
