    There's an initial report out on this. Apparently #4 reverser was locked out, and they only used 2 & 3. That's normal. But, somehow or other, they've managed to push the #4 thrust lever up from idle, achieving 109%. That explains going off the runway, but how you push one lever to the stops...

    ATC have given him a "roll through". Basically they've told him that he doesn' t need to be off the runway in a hurry, so he can be very gentle about slowing down. It's quite common, even on bigger aircraft, except that you don't really notice it on them. May not have needed any braking at all...

    A failure of the wing box would lead to the wings clapping. This seems a bit like the US Marine aircraft that was lost after a prop chopped its way through the fuselage. Nasty, no matter the cause.

    Even more noteworthy, but neither crew selected TO/GA when they realised things looked wrong. In the case of MAS, the captain didn’t because “he didn’t want to startle the FO”. I suspect he would have been even more startled if they’d run into something.

    From an MD-11 person, apparently the inhibit on the warnings that exists from V1 gets rid of the visuals, but not the fire bell. That's strange anyway, 'cos a bell going off without the associated lights would be confusing, which is why the inhibits exist in the first place.
