Mine finally arrived on the mail just now. I suggest that you keep bugging them via Document Centre or the suggested email that you mentioned. It took me 3 tries before someone remotely capable took care of the request.

My guess is that the customer services representative that called you had no clue on how to process the form (they seem to be instructed that Additional Cards are no lo her offered since March 2021). FYI. Finally after a few tries uploading the form via Document Centre I can see that the A/C card shows in the Statement --> Your Card Activity section (at the end): Now I just ahve to wait for it to arrive in the mail. Cheers

Did they process it for you? I had to call them after almost two weeks and they keep saying that they don't offer it since March 2021. This is after uploading the form via Document Centre and getting no answer.
