Like a lot of people I find it hard to sleep with aircraft noise and find that the standard foam earplugs fall out constantly - has anyone found any custom made ear plugs that stay in for the duration?

Foam ear plugs falling out? I reckon you're not fitting them into your ears correctly. I've used various types of foam earplugs in all sorts of applications and never had any 'fall out constantly'

squish the ear plugs between your thumb and first finger until its really skinny. then fit it into your ear by taking your (for example) left hand, reach over your head and grab and pull the top of your right ear lobe. once you've got hold of that, place the squished ear plug all the way in your ear and release your ear lobe. Ear plug will expand and be fitted inside your ear.

They don't do all that much anyway (IME).... There are a number of DIY molded earplug kits available now...... or you can have them made of course.... I wouldn't use anything else...

Roll the plug between finger tips to a thin profile then insert into ear.

Hold (keep in) with a fingertip for 30 seconds or so while it expands.

Remove fingertip. The ear plug should stay in place. If it doesn't, then repeat.

Works well for me.

I use a commercial pair like this that I got from a factory.

3M™ Reusable Ear Plugs Corded 1270

Unsure if you can get retail. Maybe ebay.
The cords are great on a plane so you don't lose tgen, and easy to remove

Plenty on eBay - search: 3M corded ear plug

I suffer from very bad tinnitus and worry about using ear plugs and noise cancelling headsets because of the cabin pressure. Could be over thinking it though.

I recently forgot earplugs that I routinely carry in my toiletries bag. Went to chemist and bought a pack of 3 pairs 'Hush-a-Foam' for $4.

Didn't want to pay that, but needed the plugs for what was going to be a noisy night (known barking dog next door).

Worked really well; 'smaller' and softer type, but fitted very comfortably and blocked ambient noise completely. Rated 26dB "Class 5",
