I have successfully refilled those giveaway tubes of toothpaste. Not too difficult or messy if you take care. Any " $2" shop, found in most malls, sells 100 ml bottles.

As far as I am aware they can vary the meals, but only if the components are in the relevant menu.
You are correct if London is your primary departure point, as once upon a time it was for me, but if you have connecting flights and need to negotiate the terminals, it can be challenging.
I did mean no QR. But how does that work now? I recall that during Covid some Melbourne restaurants used a QR code to link to a menu from which you ordered, so limited staff contact.
